What is Needed to Request a Copy of a Birth Certificate?
- A completed Document Request Form (PDF).
- Requester must show a copy of their state issued picture ID.
- If requesting a Birth Certificate for someone other than yourself, copies of Birth Certificates can only be given to the following relatives.
- Mother
- Father
- Brother
- Sister
Cost of Obtaining a Copy of a Birth Certificate
- $25 for the first copy
- $5 for each additional copy of the same birth certificate

What is Needed to Request a Copy of a Death Certificate?
Anyone can request a copy of a death certificate; however, the cause of death and the social security number will be redacted on death certificates given to anyone other than an immediate family member (mother, father, brother, sister or child over 15)
