The application must be completed in full, printed and signed.
Additional Requirements
The following documents are also required to be uploaded to the Department of Revenue's website at the same time as the application. Applications are not considered complete without ALL documents.
- Cover letter on the charitable organizations letterhead signed by the applicant.
- A copy of the nonprofit authorization letter (Federal Form 501-C)
